Waiver of Liability
It is the purpose of this agreement to exempt waiver and relieve releases from liability for
personal injury, property damage, and wrongful death, including if caused by negligence,
including the negligence, if any of releases ŇReleasesÓ include Stride Right Power Skating, Inc.
dba T.E.A.M.S Training and The Goalie Academy of Boston, its affiliate associations, local
associations, and member teams, event hosts, other participants, coaches, officials, sponsors,
advertisers, and each of them, their officers, directors, agents and employees.
In consideration of being permitted to enroll and participate in Stride Right Power Skating, Inc.
dba T.E.A.M.S Training and The Goalie Academy of Boston, I hereby voluntarily assume all risks
of accident or injury to my person or property, whether foreseen or unforeseen. I hereby release
Stride Right Power Skating, Inc. dba T.E.A.M.S Training and The Goalie Academy of Boston, its
employees, agents and representatives from any claim, liability, demand or suit of any kind sustained,
whether or not caused by the negligence of Stride Right Power Skating, Inc. dba T.E.A.M.S Training
and The Goalie Academy of Boston, itŐs employees, agents and representatives. By signing this release,
I certify that I have read and fully understand the conditions herein provided. In addition, I give my
permission to have my child/childrenŐs photograph/video published on Stride Right Power Skating, Inc.
dba T.E.A.M.S Training and The Goalie Academy of Boston official web site and other literature.
